OBJECTIVES: The aim of this study was to examine the associations of unprocessed red meat and processed meat consumption with cardiovascular disease (CVD) incidence and mortality, and the dose-response relationship. METHODS: Published literature was retrieved through a structured search of 10 electronic databases: MEDLINE/PubMed, Scopus, SciELO, LILACS, ScienceDirect, Web of Science, Cochrane (CENTRAL), WHOLIS, PAHO and Embase, without language or year of publication restrictions. In addition, we searched the references of published studies. This systematic review was performed in accordance with the Preferred Reporting Items for Systematic Reviews and Meta-Analyzes: The PRISMA Statement. RESULTS: Twenty-one prospective cohort studies were included in the systematic review. The CVDs evaluated in the inserted studies were stroke, heart failure (HF) and coronary heart disease (CHD). Considering the heterogeneity found in the studies, for the meta-analysis, 9 articles were included. The results presented in the meta-analysis of the association of consumption of unprocessed red meat and CVD indicated that there was a significant association with total stroke incidence (RR 1.10; 95%; CI 1.01 to 1.19; p = 0.02). There was no association with Ischemic stroke incidence, nor CHD Mortality with consumption of unprocessed red meat. However, for Hemorrhagic Stroke Mortality the assessment in the consumption of unprocessed red meat showed an association of protection for women (RR 0.64; 95%; CI 0.45 to 0.91; p = 0.01). As for the results of the meta-analysis of the association between consumption of processed meat and CVD, they indicated that there was a significant association with total stroke incidence (RR 1.17; 95%; CI 1.08 to 1.26; p < 0.0001). There was no association with Ischemic stroke, nor with CHD Mortality with consumption of processed meat. Some studies that showed no association of risk, presented a significant linear trend dose response for the association of the consumption of unprocessed red meat (Bernstein et al. 2010; Nagao et al. 2012) or processed meat (Bernstein et al. 2012) and CVD. CONCLUSION: According to the results found in the meta-analysis, the consumption of unprocessed red meat and processed meat are associated with the incidence of stroke, however, no positive association was observed in relation to mortality from CVD. This systematic review and meta-analysis protocol was registered on the PROSPERO (number: CRD42019100914).

Background: obesity can influence vitamin D levels, which in turn might be associated with cardiometabolic risk factors.Objectives:this study assessed the association between 25-hydroxyvitamin D [25(OH)D] levels and cardiometabolic risk factors in adolescents with overweight living in a region of northeastern Brazil.Material and methods: a cross-sectional study was carried out by non-probabilistic sampling in adolescents diagnosed with overweight or obesity. The subjects were divided according to their 25(OH) D status into two groups: sufficient vitamin D and hypovitaminosis D. Biodemographic, lifestyle, cardiometabolic, and biochemical factors were evaluated. A logistic regression model was applied to determine the predictors of hypovitaminosis D.Results: we found a high frequency of hypovitaminosis D (45.6 %) in adolescents. Weekly sun exposure was negatively associated with hypovitaminosis D (OR = 0.96; 95 % CI: 0.92-0.99), while significant positive associations were observed between hypovitaminosis D and blood pressure above the 95th percentile (OR = 4.00; 95 % CI: 1.19-13.37), body weight (OR = 1.04; 95 % CI: 1.01-1.07), and fasting insulin (OR = 1.13; 95 % CI: 1.05-1.22).Conclusion: hypovitaminosis D showed a high prevalence in adolescents with overweight living in a sunny region of northeastern Brazil, and cardiometabolic risk factors such as systemic arterial hypertension, high body weight, and hyperinsulinemia are predictors of hypovitaminosis D (AU)


Introducción: la obesidad puede influir en los niveles de vitamina D, lo que a su vez podría estar asociado con factores de riesgo cardiometabólico.Objetivos: este estudio evaluó la asociación entre los niveles de 25-hidroxivitamina D [25(OH)D] y los factores de riesgo cardiometabólico en adolescentes con sobrepeso que viven en una región del noreste de Brasil.Material y métodos: se realizó un estudio transversal mediante muestreo no probabilístico con adolescentes diagnosticados de sobrepeso u obesidad. Los sujetos se dividieron según su estado de 25(OH) D en dos grupos: suficiente vitamina D e hipovitaminosis D. Se evaluaron factores biodemográficos, de estilo de vida, cardiometabólicos y bioquímicos. Se aplicó un modelo de regresión logística para determinar los predictores de la hipovitaminosis D.Resultados:encontramos una alta frecuencia de hipovitaminosis D (45,6 %) en los adolescentes. La exposición semanal al sol se asoció negativamente a la hipovitaminosis D (OR = 0,96; IC 95 %: 0,92-0,99), mientras que se observaron asociaciones positivas significativas entre hipovitaminosis D y presión arterial por encima del percentil 95 (OR = 4,00; IC 95 %: 1,19-13,37), peso corporal (OR = 1,04; IC del 95 %: 1,01-1,07) e insulina en ayunas (OR = 1,13; IC del 95 %: 1,05-1,22).Conclusión: la hipovitaminosis D mostró una alta prevalencia entre los adolescentes con sobrepeso que viven en una región soleada del noreste de Brasil, y los factores de riesgo cardiometabólico, como hipertensión arterial sistémica, peso corporal elevado e hiperinsulinemia, son predictores de hipovitaminosis D (AU)

A doença causada pelo novo coronavírus (COVID-19) é atualmente o mais grave problema de saúde pública no mundo. Considerando a magnitude da doença, bem como os impactos psicossociais e econômicos, avaliar a distribuição temporal e os fatores associados com a mortalidade por COVID-19 em públicos distintos pode fornecer evidências sobre a dinâmica temporal e os grupos com maior risco de óbito. Com as medidas adotadas para o enfrentamento da pandemia, foram verificadas mudanças na prática de profissionais de saúde e professores, sendo importante avaliar o impacto na saúde mental. Nesse contexto, o objetivo deste estudo foi avaliar a morbidade e mortalidade por COVID-19 no Brasil e discutir evidencias sobre impacto da pandemia na saúde mental de profissionais de saúde e professores. Para responder a este objetivo, foram desenvolvidos quatro estudos. No primeiro, de corte transversal, foram incluídos 8.402 crianças e adolescentes de 0 a 19 anos com Síndrome Respiratório Aguda Grave (SRAG) por COVID-19. A taxa de incidência e de mortalidade foi 13,74 e 1,35 por 100.000, respectivamente, com letalidade de 9,81%. Na análise multivariada pela Regressão de Poisson com variância robusta, os fatores associados com maior risco de óbito foram faixa etária < 1 ano, cardiopatia, doenças imunossupressoras, doenças neurológicas e síndrome de Down. No segundo artigo, de revisão sistemática, foi identificado que os instrumentos Mini Nutritional Assessment (MNA), MNA-short form, o Geriatric Nutrition Risk Index, o Nutritional Risk Screening 2002, o Malnutrition Universal Screening Tool, o modified Nutrition Risk in Critically Ill (NUTRIC) score e o Subjective Global Assessmen apresentam elevada sensibilidade para identificação de risco nutricional em idosos com COVID-19. No terceiro artigo, de revisão sistemática sobre a saúde mental de professores em tempos de COVID-19, foi verificado que a prevalência de ansiedade variou de dez a 49,4%, para depressão, de 15,9 a 28,9%, e, para estresse, de 12,6 a 50,6%. Atuar em escolas (em comparação com universidades), ser do sexo feminino e estar vivenciando o retorno às aulas presenciais foram fatores relacionados com maior risco de alterações emocionais. No quarto artigo, uma revisão sistemática com metanálise sobre a ansiedade em profissionais de saúde, foi identificada prevalência de 35% (IC95%: 29-40), sendo maior nas mulheres e nos enfermeiros. Atuar na linha de frente, estar infectado com coronavírus e apresentar doenças crônicas também foram fatores associados com maior risco de ansiedade. Os resultados desta tese evidenciam que apesar de menos prevalente que em idosos, a COVID-19 também acomete considerável quantidade de crianças e adolescentes no Brasil, com maior razão de prevalência para óbito nos que apresentam cardiopatia, doenças imunossupressoras, doenças neurológicas e síndrome de Down. Para os idosos, foi verificado que os instrumentos de triagem nutricional são úteis para a identificação de risco nutricional. Foi registrada elevada prevalência de alterações do estado emocional em profissionais de saúde e professores, havendo a necessidade de medidas em saúde coletiva que visem à prevenção destas alterações nestes profissionais. Há a necessidade de inclusão desta pauta em Vigilância em Saúde, especificamente Saúde do Trabalhador (AU).


The disease caused by the new coronavirus (COVID-19) is currently the most serious public health problem in the world. Considering the magnitude of the disease, as well as the psychosocial and economic impacts, assessing the temporal distribution and factors associated with mortality from COVID-19 in different publics can provide evidence about the temporal dynamics and the groups with the highest risk of death. With the measures adopted to face the pandemic, changes were observed in the professional practice of health professionals and teachers, and it is important to assess the impact on mental health. In this context, the aim of this study was to assess morbidity and mortality from COVID-19 in Brazil and the impact of the pandemic on the mental health of health professionals and teachers. To respond to this objective, four studies were developed. In the first, a cross-sectional study, 8,402 children and adolescents aged 0 to 19 years with Severe Acute Respiratory Syndrome (SARS) due to COVID-19 were included. The incidence and mortality rates were 13.74 and 1.35 per 100,000, respectively, with a mortality rate of 9.81%. In the multivariate analysis using Poisson Regression with robust variance, the factors associated with a higher risk of death were age group < 1 year, heart disease, immunosuppressive diseases, neurological diseases and Down syndrome. In the second article, a systematic review, it was identified that the instruments Mini Nutritional Assessment (MNA), MNA-short form, the Geriatric Nutrition Risk Index, the Nutritional Risk Screening 2002, the Malnutrition Universal Screening Tool, the modified Nutrition Risk in Critically Ill (NUTRIC) score and the Subjective Global Assessmen have high sensitivity for identifying nutritional risk in older adults with COVID19. In the third article, a systematic review on the mental health of teachers in times of COVID-19, it was found that the prevalence of anxiety ranged from ten to 49.4%, for depression, from 15.9 to 28.9%, and, for stress, from 12.6 to 50.6%. Working in schools (compared to universities), being female and experiencing a return to face-to-face classes were factors related to a higher risk of emotional changes. In the fourth article, a systematic review with meta-analysis on anxiety in health professionals, a prevalence of 35% (95%CI: 29-40) was identified, being higher in women and nurses. Working on the front lines, being infected with the coronavirus and having chronic illnesses were also factors associated with a higher risk of anxiety. The results of this thesis show that despite being less prevalent than in the elderly, COVID-19 also affects a considerable number of children and adolescents in Brazil, with a higher prevalence ratio for death in those with heart disease, immunosuppressive diseases, neurological diseases and Down syndrome. A high prevalence of alterations in the emotional state was registered in health professionals and teachers, with the need for collective health measures aimed at preventing these alterations in these professionals. There is a need to include this agenda in Health Surveillance, specifically Occupational Health (AU).

BACKGROUND: Red and (particularly) processed meats are high in cholesterol and saturated and solid fatty acids. Their consumption is considered one of the risk factors for metabolic disorders. Numerous studies demonstrated a possible association between red meat consumption and cardiovascular disease (CVD). In this protocol, we propose a systematic review of the literature to examine the associations of red meat consumption with CVD incidence and mortality, and explore the potential dose-response relationship. METHODS: We will search MEDLINE/PubMed, Scopus, SciELO, LILACS, ScienceDirect, Web of Science, Cochrane (CENTRAL), WHOLIS, PAHO, and Embase. We will include prospective epidemiological studies (longitudinal cohort). Risk of bias will be assessed using the Newcastle-Ottawa scale (NOS). Four independent researchers will conduct all evaluations. Disagreements will be referred to a fifth reviewer. We will summarize our findings using a narrative approach and tables to describe the characteristics of the included studies. The heterogeneity between trial results will be evaluated using a standard chi-squared test with P < .05. We will conduct the study in accordance with the guideline of the Preferred Reporting Items for Systematic Review and Meta-analyses Protocols (PRISMA-P). RESULTS: This review will evaluate the association between red meat consumption and incidence of CVD and mortality (primary outcome measures). The secondary outcome measure will include the dose-response effect. CONCLUSION: The findings of this systematic review will summarize the latest evidence of the association between red meat consumption and incidence of CVD and mortality and the dose-response effect through a systematic review and meta-analysis. The scope of this study was to conduct a systematic review to establish the major dietary habits of adolescents and the corresponding association with cardiovascular risk factors. Research was performed in the LILACS, SciELO, PubMed, Scopus and Web of Science databases in addition to a manual search for original articles published between 2005 and 2014. Of the 371 entries identified; 26 fulfilled the inclusion criteria and were covered in the review. The main dietary habits observed were Western (61%) Healthy (42%) and Traditional (38%). Western dietary habits were positively associated with total cholesterol and SM, while Healthy dietary habits were inversely associated with fasting glycaemia, DBP and SM and positively with HDL-C, all of the aforementioned associations with very low (D) quality of evidence in accordance with the GRADE rating. The Traditional dietary habits were considered a risk factor for hyperglycemia, hyperinsulinemia and hypertriglyceridemia and protection for physical activity and eutrophic BMI. Although these results need to be analyzed with caution, due to the low quality of evidence, there is a clear need for actions aimed at promoting healthy dietary habits in adolescents in order to contribute to the prevention of cardiovascular risk factors.

Resumo O objetivo deste estudo foi identificar por meio de uma revisão sistemática os principais padrões alimentares de adolescentes e verificar sua associação com fatores de risco cardiovascular. Foi realizada busca nas bases de dados Lilacs, SciELO, PubMed, Scopus e Web of Science, além da busca manual de artigos originais publicados entre 2005 a 2014. Dos 371 registros identificados; 26 preencheram os critérios de inclusão e foram inseridos na revisão. Os principais padrões alimentares observados foram Ocidental (61%), Saudável (42%) e Tradicionais (38%). O padrão Ocidental foi positivamente associado com colesterol total e Síndrome Metabólica (SM), enquanto que o Saudável associou-se inversamente com glicemia de jejum, Pressão Arterial Diastólica (PAD) e SM e positivamente com HDL-c, todas estas associações com qualidade da evidência D, muito baixa, segundo o sistema GRADE. Os padrões Tradicionais se relacionaram como fator de risco para hiperglicemia, hiperinsulinemia e hipertrigliceridemia e proteção para atividade física e IMC eutrófico. Por mais que estes resultados precisem ser analisados com prudência, devido à baixa qualidade da evidência, observa-se a necessidade de medidas que visem à promoção da alimentação saudável em adolescentes, de modo a contribuir para a prevenção de fatores de risco cardiovascular.
